Psoriasis is a common, chronic, scaly rash that affects people of all ages (about 2% of the population) and it tends to run in families. Psoriasis is also influenced by many environmental factors. It is not contagious and is not due to an allergy.

The most common ages for psoriasis to first appear are in the late teens and in the 50s. It affects men and women equally, although in children, girls are more commonly affected than boys.

About 5% of those with psoriasis will also develop joint pains (psoriatic arthritis), which may involve one or more joints. This can be very debilitating.

What does Psoriasis look like?

Psoriasis is characterised by red, scaly patches of skin, which usually have very well defined edges. It is often symmetrical, affecting both sides of the body. The scale is typically silvery white. Psoriasis can affect any area of skin and it can sometimes be severe enough to require hospitalisation.

What causes Psoriasis?

Exactly what causes psoriasis is not fully understood but there is a lot of active research into this area. The immune system is involved and appears to be overactive in a way that causes inflammation. Specifically, there is excessive production of TH1 cytokines, particularly TNFα.

These have many effects, including growth of extra blood vessels within the skin (causing the red colour) and increased turnover of the skin cells (causing the scaling and thickening of the skin).

Genetic influences

Like most diseases, psoriasis is influenced by inherited  characteristics. Up to 50% of people with psoriasis will know of another affected family member. Patients with a family history of psoriasis tend to develop psoriasis earlier in life than those without a family history. Other factors that can trigger psoriasis includes stress, infections and injury to the skin.

Many psoriasis studies have noted many affected patients to be overweight or obese. Psoriasis is
associated with a greater risk of ischaemic heart disease (angina and heart attack), stroke and peripheral vascular disease.

Is there a cure?

There is to date no cure for psoriasis but satisfactory control of the disease is possible for most patients. Most forms of psoriasis run a fluctuating course with periods of marked improvement and even complete clearance, only to relapse at a later time. There may be several months or years between relapses. A small group of patients have severe, persistent psoriasis that is very difficult to treat and can be very disabling.

Treatment includes topical creams, oral medications, phototherapy and newer immune-modulating drugs (commonly known as biologics).

Alcohol and smoking — There is an association between increased alcohol intake and smoking and the development of severe psoriasis.

Molluscum Contagiosum

Molluscum contagiosum is caused by a pox virus and involves the outer layer of the skin, causing tiny white bumps. The centre of the bumps may have a depression filled with while material.

Molluscum contagiosum is contagious and can be transmitted to other areas of the skin or to other children. It can spread via skin to skin contact and sharing of clothes and towels. It is advisable to avoid swimming if  your child has molluscum. Infants and toddlers are more susceptible to catching molluscum. Usually, it causes no harm.
 
Your doctor may try to treat the molluscum by pricking, freezing or applying a medicated cream (like imiquimod) to the surface. Left untreated, molluscum may last 2-3 years. It clears when immunity to the virus develops.
 


Viral Warts

Warts are growths on your skin caused by an infection with human papilloma virus, or HPV. 

Types of warts include:

 
  • Common warts, which often appear on your fingers, toes and on the knees
  • Plantar warts, which show up on the soles of your feet
  • Genital warts, which are a sexually transmitted disease
  • Flat warts are skin-coloured and can appear in any area of the body
  • Periungual warts prefer to grow at the sides or under the nails and can distort nail growth
Warts are contagious and may spread from one area of the body to another or to others. There is no way to prevent warts.
 
In children, warts often go away on their own. In adults, they tend to stay. If they hurt or bother you, or if they multiply, you can remove them.
 
Treatment
 
Many people don’t bother to treat them because treatment can be more uncomfortable and troublesome than the warts – they are hardly ever a serious problem. However, warts may be painful and they often look ugly and cause embarrassment.
 
 

Cryotherapy

Cryotherapy is the most common treatment for warts. The wart is frozen with liquid nitrogen repeatedly, at one to three week intervals. This is uncomfortable for a few minutes and may result in blistering for several days.
 
Chemical treatment
 
Chemical treatment includes salicylic acid, 5-fluorouracil and imiquimod which work by removing the dead surface skin cells.
 
 

Electrosurgery

Electrosurgery is used for particularly large and annoying warts. Under local anaesthetic, the growth is pared away and the base burned by diathermy or cautery. The wound heals in about two weeks.
 
Laser treatment
 
Large warts can also be treated by CO2 laser. This can result in a wound which can take about 2 weeks to heal.

 

Impetigo

Impetigo is a skin infection caused by bacteria. It develops if the bacteria, most commonly Staphylococcus or Streptococcus, gets into the skin. Impetigo is more common when the skin has a poor immune function or if the immune system is impaired. It is occurs more commonly in children with eczema. It can appear as painful blisters , sores or yellow crust.
 
Treatment
 
Impetigo can be treated with oral antibiotics. See your doctor and complete the course of antibiotics. Children should not share towels with others and they should be encouraged to wash their hands frequently.
 
As impetigo can spread to others, affected children should be kept away from school until their lesions have recovered.

 

Candidiasis

 
Candidiasis is a skin infection caused by a fungus. It affects the body folds like the diaper area where the skin is moist and warm. It appears as red areas with small blisters.
 
Treatment
 
For infants wearing diapers, frequent changes are recommended and try to keep the area dry. Your doctor will prescribe an anti-fungal cream and use it 2-3 times a day until the rash clears.
 
 

Scabies

 
Scabies is a skin infection caused by the scabies mites. It presents as red bumps and can cause itch particularly on the hands, skin folds, abdomen and genitalia. It is transmitted from humans and often occurs within families especially when there is close contact.
 
Treatment
 
Your doctor will prescribe a lotion which get rid of the mites. All family members who have been in close contact with the affected child should also be treated. Repeat treatment 1 week later may be needed and it is important to have a follow up visit with your doctor to confirm that the treatment had been successful.

Insect bites and stings can be simply divided into 2 groups: venomous (poisonous) and non-venomous.

A sting is usually from an attack by a venomous insect such as a bee or wasp, which uses this as a defence mechanism by injecting toxic and painful venom. Whereas non-venomous insect bites pierce the skin to feed on your blood. This usually results in intense itching, which can be prolonged in some instances.

What are the signs and symptoms?

For most people, insect bites or stings cause a mild reaction. Venomous stings usually cause a stinging sensation or pain with redness and swelling of the area. Itch is usually not a concern. In some people who are sensitive to insect venom, a sting may cause a severe allergic reaction known as anaphylaxis.

This results in facial swelling, difficultly breathing and an itchy rash over most of the body. This can be life-threatening so immediate medical attention and treatment needs to be sought.

Non-venomous insect bites usually cause little more than an intense irritating itch for most people. The bite may show up as a small raised red spot. It may blister. Unfortunately the urge to scratch usually results in an open sore that may become infected and take longer to heal.

What is the treatment of insect bites and stings?

If the reaction is mild, insect stings should be treated by first removing the stinger. The site should be cleaned with a disinfectant and an ice or cold pack applied to reduce pain and swelling. If an insect sting causes a severe reaction or anaphylaxis, urgent medical attention should be sought. The main treatment aim of insect bites is to prevent itching. Topical and oral antihistamines, calamine lotion, and topical local anaesthetic agents may provide relief. Generally, as only brief treatment is required, moderate potency topical steroids may be used to provide a faster effect.

How does a pulsed dye laser work?

Pulsed Dye Lasers deliver an intense but gentle burst of light into selectively targeted areas of the skin. The light is absorbed by specific blood vessels or melanin pigmented areas in the dermis depending on the condition being treated. Factors such as the age, color and type of lesions, as well as the location on the body, all determine whether lesions can be removed.
 
 

What is the procedure like? Is there any downtime?

 
Each treatment takes about 30min. No anaesthetics are required, but they may be used. A protective eye shield is then put on for you. The feeling most patients report is a warming or tingling sensation during treatment. The skin being treated is additionally protected by Candela’s dynamic cooling method, DCD, which delivers a cooling mist on to the skin before the laser pulse is emitted. This cooling mist increases comfort during treatment, as well as protects the skin to minimize side effects like redness. There is no downtime after the treatment – you can apply makeup immediately after if needed and continue with your normal activities
 
 

Is PDL Treatment safe?

 
Yes. In fact, treatment is so gentle and safe that it has been successfully used since the 1980’s for the treatment of port wine stain birthmarks in infants and young children, before they are old enough to face harsh social consequences. During treatment, the epidermis is protected by Candela’s exclusive dynamic cooling method, DCD, which sprays a cooling mist onto the targeted area of skin before each laser pulse, maximizing comfort and protecting the skin during treatment.
 
 

How many treatments are needed?

The number of treatments will depend on :
 
  • Desired degree of improvement
  • Your hormonal status
  • Size of treatment area
  • Pre-medication

On average, treatment can be done 2-4 weeks. For removal of broken capillaries or veins, you may need 2-4 sessions. For clearance of red acne scars, you may need approximately 6 or more sessions for optimal effect. You will be advised further upon consultation with the doctor.

Are there any skin types that are not advised to be treated?

The PDL laser is best suited for lighter skin types or untanned skins. Speak to our doctor to find out if you are suitable for this laser treatment. 
 
 

What side-effects can I expect and how long will they last?

 
You will have some degree of redness and swelling, but it should resolve within a few hours 
after treatment. The treated areas may appear darker and can bruise for a short period of time.  
This is normal and will resolve in a few days.
 

Are there any special precautions I need to take before or after treatment?

It is recommended to avoid sun exposure for 4 – 6 weeks before and throughout treatment to maximize results. A sunblock of SPF 30 or higher should be worn daily during this time. Post treatment care guidelines depend on then condition treated, and should be discussed with you practitioner.

How does Gentle Hair Removal treatments work ?

What areas of the body can be treated?

Are the treatments painful?

While other Laser hair removal gives a prickly or a light rubber-band snapping sensation, the Candela’s unique . Gentle Hair Removal treatments use a patented Dynamic Cooling Device™ (DCD™), which prepares the skin for treatment by spraying the upper layer with a cooling burst of cryogen in adjustable durations—milliseconds before the laser pulse. DCD helps ensure that Gentle Hair Removal treatments are as comfortable as they are effective.

How long does each treatment take?

An upper lip and chin can be done in 5 minutes. Underarms or bikini lines usually take 10-15 minutes.
Legs and back take longer.

Is the hair removal permanent and how many treatments do I need?

The treated hair will not fall out until 2 weeks later. Mild redness and swelling after the treatment is expected. These reactions subside within few hours to 1 – 2 days. Hair growth is reduced after each treatment and the number of treatments required will be based on your hair color and type, body
area and skin tone. The best results are achieved after a series of four to eight treatments at four to eight week intervals depending on area treated. The process is most effective when the hair follicle is in its active growth phase.

What can I expect and how to take care of my skin after laser hair removal?

Patients scheduled for hair removal should avoid plucking, electrolysis or hair waxing for six weeks prior to treatment. If you have had a history of herpes simplex virus and are scheduled for facial hair treatment, prophylactic antiviral therapy may be prescribed. .

During or following treatment, a cool compress or gel pack may be applied for comfort. Any redness or swelling should resolve within a few hours.

After laser treatment, prevent sun exposure by applying sunblock (SPF 30 or above). In the first few  days after treatment, don’t scrub or rinse the treated areas with abrasive skin cleansers. Your practitioner may advise additional precautions.

Are there any possible side effects?

Side effects while few and rarely permanent may include redness, swelling, hyperpigmentation, hypopigmentation, blistering and purpura (a laser bruise) in the treated area—these usually do not last more than a few days. Skin lightening (hypopigmentation) is rare, but darkening (hyper-pigmentation) may occur, especially in patients with darker skin tones. These skin colour changes will fade with time.

How does the MiXto SX Micro Fractional CO2 laser work?

The MiXto fractional CO2 laser is a revolutionary delivery system that works by delivering controlled heat to the skin. The depth of penetration and amount of heat delivered can be controlled precisely through a high speed scanner, driven by an algorithm that divides the treatment area into four quadrants (or fractions), and skips the beam from quadrant to quadrant so that each strike is separated by the longest possible interval.

How does the treatment feel like?

Most patients will feel a warm sensation on the skin. This is expected and topical anaesthetic cream will be applied on the skin an hour before the treatment.

How long is the treatment?

Treatment time for a full face is approximately 25 minutes.

How would my skin look like after treatment? Any post care treatment I should be aware of?

Day 1 – 2

The skin will look red and sunburnt for 2-3 days post treatment. The fine pinpoint pattern will begin to disappear and the skin will have a tanned look. At this stage, makeup can be applied. Most patients return to their normal daily routine.

Day 3 – 4

The top layer of skin will begin to peel during normal washing, revealing new rejuvenated skin with improved colour and texture. During this period, let any skin to peel off naturally, and do not aggravate the process.

Day 5 – 6

The peel with will be complete in most cases. Avoid prolonged sun exposure for at least 3 months. Apply a moisturizing sun block with at least SPF30 daily.

How many treatments are required?

Superficial wrinkles and skin lesions typically disappear after one treatment. However, deeper wrinkles and acne scars may need 2 to 3 treatments, spaced about 1 month apart.

What are the risks and side effects?

Although side effects are uncommon, one should note the following:

  • Erythema (redness of skin): the laser-treated areas have a distinctive redness representing increased blood flow due to the growth of the superficial tissue and collagen. It will gradually fade during the
    first few days.
  • Inflammation (swelling): there is a minimal amount of swelling that should subside within 24 hrs.
  • Hyperpigmentation (tanning): This may occur after the 3rd day of procedure but begins to shed and reveal younger healthier looking skin.
    If you are taking any medications, you should reveal so during your doctor’s consultation. Also, if you are prone to cold sores, you may need to ask your doctor for an antiviral prescription before the treatment.
