Sweat glands act as the body’s natural thermostat. When they suddenly go into overdrive, you get sweaty palms and feet. While this can be socially embarrassing, the good news is that excessive sweating, or hyperhidrosis, can be controlled with treatment.

Vitiligo on Adults

Vitiligo 

Vitiligo is a disease that causes the loss of skin color in blotches.The extent and rate of color loss from vitiligo is unpredictable. It can affect the skin on any part of your body. It may also affect hair and the inside of the mouth.

Normally, the color of hair and skin is determined by melanin. Vitiligo occurs when the cells that produce melanin die or stop functioning. Vitiligo affects people of all skin types, but it may be more noticeable in people with darker skin. The condition is not life-threatening or contagious. It can be stressful or make you feel bad about yourself.

Treatment for vitiligo may restore color to the affected skin. But it does not prevent continued loss of skin color or a recurrence.

Symptoms

The main sign of vitiligo is patchy loss of skin color. Usually, the discoloration first shows on sun-exposed areas, such as the hands, feet, arms, face and lips.

Vitiligo signs include:

  • Patchy loss of skin color
  • Premature whitening or graying of the hair on your scalp, eyelashes, eyebrows or beard
  • Loss of color in the tissues that line the inside of your mouth and nose (mucous membranes)
  • Loss of or change in color of the inner layer of the eyeball (retina)
  • Vitiligo can start at any age, but often appears before age 20.

Depending on the type of vitiligo you have, the discolored patches may cover:

  • Many parts of your body. With this most common type, called generalized vitiligo, the discolored patches often progress similarly on corresponding body parts (symmetrically).
  • Only one side or part of your body. This type, called segmental vitiligo, tends to occur at a younger age, progress for a year or two, then stop.
  • One or only a few areas of your body. This type is called localized (focal) vitiligo.

It’s difficult to predict how your disease will progress. Sometimes the patches stop forming without treatment. In most cases, pigment loss spreads and eventually involves most of your skin. Rarely, the skin gets its color back.

When to see a doctor

See your doctor if areas of your skin, hair or eyes lose coloring. Vitiligo has no cure. But treatment may help to stop or slow the discoloring process and return some color to your skin.

Causes

Vitiligo occurs when pigment-producing cells (melanocytes) die or stop producing melanin — the pigment that gives your skin, hair and eyes color. The involved patches of skin become lighter or white. Doctors don’t know why the cells fail or die. It may be related to:

  • A disorder in which your immune system attacks and destroys the melanocytes in the skin
  • Family history (heredity)
  • A trigger event, such as sunburn, stress or exposure to industrial chemicals

Complications

People with vitiligo may be at increased risk of:

  • Social or psychological distress
  • Sunburn and skin cancer
  • Eye problems, such as inflammation of the iris (iritis)
  • Hearing loss

Chemical Peel

Chemical Peel

A facial peel refers to the application of one or more chemicals to the face which ‘burn’ off damaged cells. 

They may be used to treat damage caused by exposure to the sun, to remove pigmentation such and fine lines and wrinkles.

A peel removes several layers of sun damaged skin cells, leaving fresh skin which has a more even
surface and colour. It may stimulate new collagen to be formed improving skin texture. Peels may result
in superficial, moderate depth or deep skin injury.
 

Uses of chemical peels:

Acne like blackheads and whiteheads and the mild superficial acne scars.
Pigmentation of the skin like sun spots, age spots, freckles and melasma. Minimise fine wrinkles especially around the eyes. It does not treat severe wrinkles.
 
There are 3 main types of chemical peels including Glycolic acid (alpha hydroxyacids), Trichloracetic acid (TCA) and Jessner peels (containing salicylic acid and lactic acid).
 

Before the peel

Pre-treatment creams are applied to the face at night for several weeks prior to the peel. By exfoliating the skin and reducing pigmentation themselves, they improve the results seen from chemical peeling.

 

The peel

Superficial chemical peels are a minor procedure and no special arrangements are needed. First the face is thoroughly washed to remove surface oil. The peeling agent is then applied for several minutes. It stings – how badly and for how long depends on the chemical, its concentration, whether you’ve had pre-treatment with aspirin, and individual factors. A fan can help. The peel is then neutralised, and the burning sensation lessens. Antibiotics and painkillers may be required after deeper peels.
 
 

Afterwards

Superficial peels result in mild facial redness and occasional swelling which usually resolve within 48 hours. The peeling is similar to sunburn. Most people can continue their normal activities. Make-up
can be applied a few hours after the procedure.
 
Looking after the skin after the peel:

  • Keep treated areas cool (use a water spray).
  • Do not pick! Picking delays healing and causes scarring
  • Moisturise – use light preparations after a superficial peel, thicker moisturisers after a deeper peel
  • Protect from the sun – especially for the first 6 months

Cafe A Lait

Café-au-lait macules (CALMs), or café-au-lait spots, are flat, oval lesions (marks) on the skin. Café-au-lait means “coffee with milk” in French. The name refers to the colour of the spots, as they are at least a shade darker than your child’s skin tone.

Café-au-lait macules commonly appear at birth but may develop later, during the first year of life. Their colour varies from light brown to dark brown.

How do CALMs affect the body?

CALMs are benign (harmless). They do not hurt or itch and never progress to anything serious such as skin cancer. The spots can become darker throughout childhood or with sun exposure, but this is not a cause for concern.

How are CALMs diagnosed?

CALMs are easily diagnosed by a skin examination. “Typical” CALMs range from light to dark brown and are well defined, even coloured and often round or oval. They vary in size and can appear anywhere on the skin, although they rarely appear on the face.

How are CALMs treated?

No therapy is needed for CALMs. If your child is concerned about how their CALMs look, you can consider covering the CALMs with camouflage make-up to make them less noticeable. Some people consider laser treatment to reduce the colour.
